JOB DESCRIPTION _____________________________________________________________________________ Summary/Objective The Peer Specialist will provide peer support services as part of a multi-disciplinary team to persons with mental illness and/or substance abuse problems.
Service provision will focus on working with clients to enhance their recovery.
Service is provided to individuals or groups.
This individual will build a productive relationship with patients and provide case management to assist with the coordination of care and transition into the community.
Essential Functions Work with clients to build a supportive and conducive environment for the client's growth and recovery.
Assist with arranging or providing transportation, manage follow-up appointments, and work with facility staff to assist clientele in treatment and recovery efforts.
Knowledge of mental health and substance use conditions, including recognizing coping strategies, including the grief process, knowing when to refer and report to leadership, and understanding the interactions between physical and behavioral health.
Work with clients in creating and implementing appropriate social activities Assure access to and/or provide necessary training, groups, and support services for clients.
Work with a clinical team to assist clients in setting and meeting treatment goals.
Develop a respectful, healthy, and trusting mentoring relationship with clients.
Assist in developing support resources necessary for the client, such as healthcare, housing, job assistance, and additional treatment services.
Document all services provided and collect/track data relating to performance goals, along with the quality assurance of services offered.
Provide support as the client transitions back into the community during recovery, including re-integrating with friends, family, and the community.
Provide administrative support for all written correspondence and scheduling for clinical needs.
Assist Clinicians with setting appointments, scheduling assessments, and intake/discharge paperwork.
Facilitate the administration of the Addiction Severity Index (ASI) assessment.
Obtain assistance/benefits for patients in a residential program to assist in successful reintegration into the community.

Physical Demands The physical demands described here represent those that must be met by an employee to perform the essential functions of this job successfully.
While performing the duties of this job, the employee is regularly required to talk or hear.
Specific vision abilities required by this job include close vision and the ability to adjust focus.
This would require the ability to lift files, open filing cabinets, and bend or stand on a stool as necessary.
Position Type/Expected Hours of Work This is a full-time position.
Days and hours of work vary according to shift.
Travel Little to no travel is expected for this position.
Required Education, Experience or Eligibility Qualifications An associate degree with a minimum of one year of peer specialist or related experience OR High School diploma or equivalent and minimum of two years of peer specialist experience or related field experience.
Peer Specialist Certification must be obtained through the next available scheduled training.
