SUMMARY:
Under the general supervision of the Transitional Rehab Director this position provides quality, professional, nursing care and medical oversight to St.
John’s Transitional Rehab Center Patients.
ESSENTIAL JOB FUNCTIONS:
Unit Clerk:
Promotes the patient’s well-being and satisfaction by supporting patient-centered care while working as a member of the Clinical Support Team.
Schedules appointments, procedures, and transportation.
Make appointments and place in patient rooms
Notify POA/Family of appointments if applicable
Cancel and reschedules appointments
Enter appointments in PCC and Sharepoint
Fill out paperwork to go with patients
Assist with completion of STI paperwork as needed
Collaborate with family and therapy for scheduling
Monitor unit oxygen – admissions, discharges and discontinuation
Collaborate with nursing staff
Fax oxygen information to BC supply
Make photocopies and fax documents as needed
Send home health, hospice, and assisted living orders on discharge
Send home health referrals
Communicate with lab and x-ray as necessary
Complete lab slips for morning draws
Call imaging with new orders
Phlebotomy services as assigned with appropriate education if applicable
Immunization management
Refrigerator temps
Daily glucometer checks
Admit and Discharge patient from the computer
Assemble, break down and manage patient charts and records
Route forms to appropriate personnel
Leadership:
Liaison with laundry, housekeeping, and culinary services
Assist with staffing as assigned
Manage call offs – send out messages for open shifts
Notify leadership of call offs, issues
Follow all facility policies and procedures
Assists staff when necessary
Works closely with physicians to determine appropriate care
Assists as directed in emergency situations
Assist visitors as necessary
Take messages for nurses, if unavailable
Contact maintenance and IT as appropriate
Order supplies and equipment
TC Stock orders from pharmacy, fax to pharmacy
Restock as appropriate
General unit organization, supply cabinets labeled
Assist staff with DME ordering and oxygen procurement on discharge
Maintain the confidentiality of all patient records
Answers the phone and directs calls
Check supply for expiration dates monthly
Communication/Reporting:
Reports pertinent issues to the Nursing Administrator on-call.
Participates in and receives the nursing report on assigned shifts
Manages on call phone while on shift.
Maintains inventory of equipment
Facilitates the delivery of equipment/supplies to the area of need.
Manipulates schedule to assure all care areas throughout campus have care coverage.
Maintains the confidentiality of all patient care information according to the St.
John’s Policy on HIPAA and Patient Rights
Requirements:
Minimum Requirements: Must be licensed as a CNA in the State of Montana.
2-5 years of experience desired.
Can be combined with other medical experience if applicable.
Knowledge, skills, and abilities required:
Ability to understand and express written and verbal communication.
Ability to interact effectively and professionally with patient’s, family members, coworkers and other clinical support staff
Ability to work independently and in a team
Ability to lift and manipulate heavy objects
Ability to understand and follow safety policies and procedures
Ability to use various health care supplies and equipment, including blood pressure devices, thermometers, wheelchairs, lifts, and oxygen suppliers, etc.
Ability to understand and to contribute to patient charts and care plans
Ability to relate to and work with the ill, elderly, disabled, emotionally upset, and at times, combative people within the facility
Ability to respond to patient needs in a timely and professional manner, recognizing the patient’s rights
Ability to deal with stressful situations (i.
e.
death and dying)
PHYSICAL DEMANDS: Requires moderate physical work more than 70% of the time, including lifting up to 25 pounds regularly and up to 50 pounds occasionally, standing, pushing, pulling, lifting, walking, and bending for prolonged periods of time.
WORKING CONDITIONS:
Work involves some exposure to dirt, dust, fumes, unpleasant odors, and/or loud noises.
Work involves some exposure to hazards, bloodborne pathogens, or physical risks, which require following basic safety precautions.
