The Licensed Practical Nurse will function in a charge nurse capacity in Residential Living Unit, under the general supervision of a registered nurse\*.
The Licensed Practical Nurse utilizes an interpersonal process that uniquely focuses on the total individual and his/her role in their family, culture and society.
The Licensed Practical Nurse uses standardized procedures in the observation and care of the ill, injured and infirm, in the maintenance of health, in action to safeguard life and health and in the administration of medications and treatments prescribed by authorized individuals.
The Licensed Practical Nurse fulfills a resident advocacy role; by ensuring the resident and/or their representative has adequate information to make health decisions; by coordinating total care; and by working with the resident to achieve or maintain their highest practicable level of function.
The Licensed Practical Nurse is guided by the substantial specialized knowledge of nursing based on the physiological and psychological sciences.
The Licensed Practical Nurse ensures that the quality of resident care meets legal and professional standards and also acts as a role model to other members of the health care team.
*Skills:*
Assessment:
• Completes holistic, age-specific admission history and physical assessment on all assigned residents in an ongoing manner to monitor for deviations.
• Analyzes and reports to appropriate individuals, clinical data (i.
e.
lab, X-ray, and all diagnostics) which may indicate a need for a change in the plan of care.
• Identifies and documents residents at risk for nutritional and functional status.
• Identifies and reports signs/symptoms of neglect/abuse related to child, domestic or elder abuse.
Planning:
• Sets priorities for and reviews daily plan of care, in collaboration with resident/family/significant other and other healthcare disciplines.
• Delegation to team members is based on resident needs, continuity of care, staff skill and knowledge.
• Communicates with physician and Registered Staff to ensure coordination of medical and nursing plan of care.
Implementation:
• Demonstrates competence in performing technical skills.
• Proactively recognizes and demonstrates ability to manage urgent/emergent resident care situations.
• Demonstrates knowledge and technical competence in preparation and administration of medications and IV’s per scope of practice and hospital policy.
Evaluation:
• Documents nursing interventions, resident’s response to care, and resident outcomes.
• Collaborates with other members of the health care team to evaluate outcomes and contributes to the revision of the resident’s care plan base on these evaluations.
Resident Education/Continuum Care:
• Identifies and documents resident/family learning needs and provides education, based on assessment in collaboration and coordination with resident, family and interdisciplinary team members.
• Identifies and documents discharge planning needs.
• Coordinates or delegates discharge activities: completes or validates transfer/discharge documents, instructions related to medications, activity, wound care and follow up appointments, etc.
• Documents resident/family response to education.
Unit Specific:
• Maintains working knowledge of CMS Regulations so that documentation supports and reflects compliance.
• Maintains working knowledge of Medicare skilled and MDS criteria so that documentation supports the need for care as well as reflects resident status.
• Incorporates resident needs and preferences into organized, efficient delivery of care by a team assigned for daily cares and delegates tasks appropriately
• Supervises, coaches, counsels, and evaluates performance of assigned C.

Required:
● Graduate of an accredited practical nursing education program
Required:
● Current license to practice as an LPN in the state of Montana
● Current BLS
Required:
● Minimum of 1 year experience as an LPN in a LTC other related health care facility.
Preferred:
● 2 years' experience as an LPN in a LTC facility.
