JOB SUMMARY Provides overall management of clinical, regulatory, administrative and financial direction of Home Health/Hospice.
Assume responsibility for all activities and personnel.
Ensures provision of services that reflect the organization’s philosophy and standards.
Adheres to all state, federal, JCAHO and other regulatory guidelines as applicable.
Evaluates provision of services and takes corrective action through a performance improvement process.
Develops policies and procedures.
Develops a system of staff communication that ensures coordinated implementation of plans of treatment and the utilization of necessary services for comprehensive care of patients.
Represents the programs in the community in a positive manner.
ESSENTIAL JOB FUNCTIONS Responsible for all aspects of the operation of Home Health and Hospice.
Plans, coordinates, and facilitates the operations of Home Health and Hospice.
Provides leadership and direction to all outreach programs in accordance with organizational goals and objectives.
Participates in strategic planning and performance improvement activities of assigned programs.
Ensures that all rules and regulations are observed by employees, to include overall corporate compliance and compliance with federal, state, HIPAA and JCAHO standards.
Keeps employees informed about their work assignment, work progress, and opportunities for advancement.
Conducts timely performance evaluations on new or probationary employees, regular employees, and employees who are being considered for termination.
Ensures a mechanism for the ongoing review of departmental policies and procedures which meet the JCAHO standards.
Develops the capital and operating budgets for assigned outreach programs.
Knowledgeable of current federal, state, JCAHO, other regulatory standards relevant to Home Health and Hospice.
Ensures departments are represented in at least one organizational Performance Improvement activity per year.
Reviews PI, Patient Satisfaction Surveys, Infection Control Reports and integrates appropriate actions.
Attends a minimum of 80% of Department Head and Clinical Management meetings per year.
Interviews prospective staff, explains duties, reviews applications and references, and hires new employees.
Provides for professional growth and development of staff through departmental orientation, identification of needs, sponsoring training programs, encouraging education (formal and continuing) efforts, and evaluating outcomes.
Counsels employees, assists directly in conflict resolution, develops staff and, as appropriate, utilizes the disciplinary process.
Develops, oversees, justifies, and maintains a fiscal plan (budget) for the assigned programs.
Monitors patient revenues, expenses and operating expenses (salaries and capital expense items) and provide appropriate recommendations and justification of variances.
Represents and interprets Home Health and Hospice functions to other institutions, the public, government agencies and other organizations.
Fosters good public relations for Home Health and Hospice.
Works with medical staff to coordinate medical and nursing management of patient care.
Performs clinical supervisory visits with each nurse and therapy service in accordance with agency policy.
Demonstrates knowledge of patient care needs.
Evaluates licensed nurses’ ability to assess and reassess patients, pain management and develop an appropriate plan of care.
Plans and conducts staff meetings.
Records, distributes and maintains minutes of meetings.
Demonstrates the ability to be flexible, organized and function under stressful situations.
Initiates the development and implementation of policies and procedures for the departments.
Presents annual reports reflecting the services and activities of Hospice and Home Health and such special reports as may be required by the Board or Administration to the Board of Trustees and staff.
Ensures that the agencies maintain accreditation, licensing and quality patient care through the establishment of performance improvement monitoring programs and standards.
Communicates effectively with patients and families and health professionals regarding care for the patients and to ensure satisfaction with services provided.
Treat patients and their families with respect and dignity.
Completes assignments accurately and on time.
Assumes the role of patient advocate.
Demonstrates knowledge of, and ensures compliance by all staff with, OSHA regulation.
Complies, and ensures compliance by all staff with Blood-borne Pathogen and TB Exposure Control Plans, and a Hazardous Waste Management program.
Demonstrates and ensures adherence by all staff to infection control policies including use of PPE.
Addresses and supports cultural practices, as long as such practices do not harm others.
Delegates responsibilities according to abilities and defined responsibilities.

POSITION QUALIFICATIONS Education / Experience / License and Certifications Minimum of 3 years supervisory experience, preferred.
Current unrestricted Wyoming Registered Nurse license, required.
Minimum of 3 years of related experience, preferred.
BSN required; Master's degree in nursing or related field, preferred.
BLS certification, required.
A reliable automobile and a willingness to operate a personal car as necessitated by the nature of the job.
Criminal and Central Registry Background Checks, required.
Valid, unexpired Driver's License, required.
Current Motor Vehicle Record (MVR) meeting the acceptable guidelines as outlined in the Drive Safe Policy, upon hire and continued throughout the duration of employment in any position that requires driving, required.
Proof of current motor vehicle insurance in the form of a policy declarations page with minimum limits of not less than $300,000 combined coverage is required.
Additional Skills Knowledge of the duties and functions of the Home Health Agency and Hospice Agency and their relationship to the healthcare community.
Comprehensive knowledge of state and federal guidelines and regulations.
